(Sorry for the belated reply. I stopped medal hunting quite a while ago and only check this thread when I take time out of my day to casually browse the forums.)

I'm always repeating this one over and over but it's very demotivating when you miss hard medals all the time because you forgot to refresh the tab after playing for more than 40? 50? minutes, and have to start from scratch.
I have seen a very straightforward solution to that problem when flash games were still running on flash and dragons ruled the twilight sky...: A [Resent Medals]-button. As the name suggests, it re-sends all the medals that the game marked as unlocked in a game-internal list. Extremely helpful.
From my perspective, the responsibility here lies with particular devs that didn't/don't care enough about their players. Medals are good enough for them to get them "extra numbers", reviews, and rewards but those devs did/do not return the favor and take proper precautions.
[...] You guys really should contact Tom and make him aware of all these issues.
I'm sure he is aware.
Just as he is aware of the million different (and very important!!) things that users just like me bring to his attention on the regular. I am 100% certain that he fully understands that we mean well - but the day I die will be cause for a little celebration in the Fulp household. ;3

I might have a suggestion, but it's really primitive and stupid. For users over 150,000(?) medal points, a little check mark appears in their Account Page that reads "Check Eligibility for joining the Top Medal Point Users List" *. As long as it's checked, the internal algorithm which created the current list will check those accounts and add them to said list - expanded to 200 users. But the numbers are only checked every Friday at midnight, so the serves don't melt over something that 99% of users do not care about.
In addition, the information "User X is a cheater" should be saved in NG's account database, so that list cannot be circumvented by abusing the name-change feature, stealing somebody's laptop, or sleeping with a certain moderator for (and I hope I translated this correctly) "base motives".

* A side benefit would be that, this way, users who want to "retire" from the list voluntarily or do not like to "show off" can do so in a quick and unbureaucratic manner - a well as change their mind in the future without bothering any moderators. Which actually brings me to something I had to bring up and have been holding off on. It seems only fitting that I bring it up in a literal footnote the way I kept neglecting this: Dear @Bobogoobo, once this list runs again (in one form or another), please no longer list me. Other things have taken priority over dealing with medals for me and whoever gets to join the list at the 200th place because I made space will probably be really happy about it. Thank you for your efforts. I'll still help out by complaining. ;)
