I have always believed that at this moment Sanford screams not so much from physical pain as from emotional pain, that there is no one else to protect his back.
Me when I get that random pain in my chest:
Me after the slightest inconvenience.
I find the scene of Sanford screaming prob has to be some of the most ... don't even know. It's like all the kick-ass madness aside, this dude is suffering, and that one scene was made just for that, we look all around realizing that it went from a guy tryna help hiss friend to him in hell, hopeless, to the brim of death while seeing the most ass-backward clusterfuck to have ever witnessed, all in a few short minutes. In short, tha shi deep.
