The scissor blade beneath the skin behind that eye... holy hell. You two make a twisted pair and this is the proof.
The strokes for shading and depth and texture (especially the wood) are so fine that they can be taken for granted, and thus we're all left having to stare those two hideous expressions. They're absolutely perfect in how they convey a shared delight and cruelty. The eye is drawn more slowly down to the dark corner of the piece, with the sad, mutilated little guy, and that effectually creates the sense of something horrific dawning slowly on the viewer. For that reason, this transcends a mere portrayal of the subject matter, and forces a genuine moment of grim discovery onto the viewer.
