My guess is Twilight is looking at Suirano breaking the 4th wall. Starlight asking if it's contagious is a play on words that Suirano will do her next.
I might be wrong tho.

Amazingly done, though I find it funny that Starlight is worried anyone could catch it. As if that expansion was like a cold, silly Starlight being silly again. Love Twilight's moderately annoyed expression, it almost seems like she expected it to happen.
